Whether you’re uninsured, have a high deductible or copay, or its simply not convenient to visit your doctor’s office, you can still experience personal, high quality care with our Telemedicine services.

Routine telemedicine visit with prescriptions as appropriate $75.00

Off work note $15.00

Lab orders & colorguard for colon CA Screening $45.00 (does not include the cost of the labs)

Radiology orders including mammogram with interpretation $45.00 (does not include the cost of the imaging studies)

Handicap placard forms or disability forms & extensions $50.00

Cancellation at least 8 hours before appointment – no charge

Cancellation less than 8 hours before appointment - $25

No show = no cancellation $50
